- Professor: MD/MS/DNB with 3 years as Associate Professor, 2 publications and biomedical research course (M.Sc+PhD routes for Microbiology/Pharmacology; Emergency Medicine and super-specialty variants per notice).
- Associate Professor: MD/MS/DNB with 4 years as Assistant Professor (5 if direct Govt appointee), 2 publications and research course.

- Retired Government/Defence faculty may apply for Professor; age and reservation per UP Government rules.
